Patients at six clinic locations completed intake forms on paper. Reception teams then entered the same information into the practice management system, taking 18 to 22 minutes per patient. Handwriting errors, missing information, and expired insurance details created more follow-up work.
Clinicians could not review a complete history before the appointment, and paper health information made consistent access controls and audit records harder to manage.
HMB Digital created a mobile-friendly pre-visit workflow. Patients receive a secure link by text message or email, complete the right forms on their phone, add insurance information, sign consent forms, and submit before arrival.
The platform sends structured information to the practice system, creates a provider summary, records activity, and gives staff an exception queue for incomplete or unusual submissions. The design uses encrypted data transfer, controlled access, and AWS services that are eligible for regulated healthcare workloads.
